Philhealth: Changing Membership Status from Employed to Self-employed

I've been a member of PhilHealth since I was employed by a private company in 2009 but since my contract was finished, I decided to change my membership coverage to self-employed.  One of the important things you have to do before separating from the company you work for is to ask HR for a copy of your appropriate registration/PIN/membership numbers for SSS, PhilHealth, Pag-Ibig etc.  Not everything can be retrieved online and it's too much of a hassle to go to each branch and fall in line and stuff.



What if I don't have a Philhealth ID but am registered to Philhealth?

If you don't have a Philhealth ID, make sure to ask your HR department for a copy of your Philhealth Membership Data Record (aka MDR) where it lists your personal PIN number, your employer's PEN number and a list of your dependents (if any) or ask from Philhealth directly (contact info posted above) or at their branches.

Checking contributions however, was a different matter and will depend on whether the contributions made by your employer are reflected in their online database.  I tried calling the Philhealth hotline 441-74-42 and was told that there is no updated record of my contributions because my company was remitting through the local branch office in Makati and she suggested that I go there myself.   Err, no, thanks.

I went to the Las Piñas office which was the branch nearest to my area.  It's the pink building (Editha Building) located across SM Southmall, beside BDO.  Proceeded directly to the second floor towards the information desk where you can get the related forms you need and they can help you with whatever it is you need to do.

Their head office is in Citystate Centre, 709 Shaw Boulevard, 1603 Pasig City.


Which forms do I need?

You can get the forms from the info desk and there are adequate tables where you can fill up the forms.  You can also download the forms at the website and bring it with you.

For amending membership coverage from employed to self-employed, use the Philhealth Membership Registration Form (PMRF ) (download here).  

Tick the box in the upper right hand corner that says "For updating."

Under Membership category in the lower half of the page, choose Self-Earning, write your profession and put in your estimated monthly income.

For those with monthly incomes below P25,000, your monthly contribution is Php 200 (600 quarterly or 2400 annually) while for those with incomes above P25,000, the monthly contribution is Php 300 (900 quarterly or 3600 annually).

They'll give you your updated MDR form and make sure to check all data in the form before payment.  You'll need to pay for either your monthly/quarterly/annually contributions for the change in coverage to take effect.  

In my case, I paid for the first quarter of  2012. at the cashier at the 3rd floor and then went back to the service counter to show the receipt.  There were about 10-15 people in line but it moved quickly since there were 2 cashiers at the counter.  Afterwards, she gave me my Philhealth ID and I'm done for the day.  Took less than 40 minutes from the time I arrived at the place.

  4. hi, im just a new philhealth member, i regstered online on january 25 2016, and they sent to me my MDR via email, but why is that my member category is indigent even if i choose individually paying? i am not yet employed as it is one of the requirements, and my surname on my MDR was mispelled. i fill up online correct spelling with my name.how can i fix this?

    1. Try calling their hotline first. Otherwise you'll need to have it corrected at a Philhealth branch nearest to your location.

    1. Download the PMR form at the Philhealth website or https://www.philhealth.gov.ph/downloads/membership/pmrf_revised.pdf

      Check "For updating" and select the new member category you're switching to (instead of Indigent). Bring your Philhealth ID and a government issued ID. They may also require you to pay for the membership dues (depending if you're on employed status).

Iam a philhealth member since 2004 iam a go ernement employee but recently upon checking my contribution my last reflected contribution is september 2018, however my payslipt has a continous monthly philhealth dediction in.addition.my employment status according to my.MDR is separated but I am very much active in the service

    1. Ah, this is where it gets tricky. Make sure maayos mo yung Philhealth mo before you leave government service (if you do decide to leave for whatever reason).

      If directly employed ka by the government, ask your HR (if meron, or its equivalent) about the discrepancy. Sabihin mo lang nag-uupdate ka ng records mo, pwede ba humingi ng proof of payment/certification sa kanila na bayad ang Philhealth contributions mo.

      Mahihirapan ka magclaim ng Philhealth benefits if hindi updated ang account mo. Is the Sept 2018 verified by Philhealth? Baka naman may sariling records yung government office mo.

      If you're employed as a contractor or if under an agency and not directly under the government, ask your agency/employer for a certification or proof that they remit your Philhealth contributions so you can have your record updated. If wala sila mapakita, magduda ka na. Good luck!
