Update: September 3, 2019
For NCR passers, you need to apply for an online appointment here if you plan to get a Certificate of Board Rating and Passing based on your profession and the year you passed (pls refer to the list here) and select PRC NCR-Morayta (Main).
If you passed earlier than the stated year in the list, you need to apply directly as "walk-in."
In my case, I needed certified true copies of my PRC itemized board rating and registration certificate (authentication) and I also decided to get a certificate of good standing as well.
Let's do this step-by step:
1. Upon entry to PRC, proceed to the window on your left to get the Authentication Form if you want to have your copies authenticated and marked as "certified true copy." For the certificate of good standing, ask for the Certification Form.
2. Buy document stamps from the window M-N, 1 stamp per document (Php 21/stamp) and affix it to your photocopies. There are some areas where you can find paste lying around, otherwise it would be handy to bring your own or borrow if you can find someone who brought theirs.
3. Proceed to the cashier and pay for the forms. Php 75/document
4. Go the respective window to submit your documents and wait for your claim slip. Make sure to include your original documents (which will be returned to you).
Window K for itemized board rating. Sign your name in the logbook.
Window 11 (inside PRC main building, right side upon entry) for PRC registration certificate and certificate of good standing.
They will indicate the date on the claim slip provided, usually 1-2 weeks depending on the type of documents.
What if I lost my original document?

ReplyDeleteYes prc id and board rating can be authenticated at any branch,as for the certificate of registration, you can actually claim it where you took your registration. Sample if you took your examination in baguio , you will probably go to baguio for your cert of registration.

ReplyDeleteYes,just present a letter authorizing your representative to transact on your behalf. They must have valid ID and a copy of your PRC id.

ReplyDeleteI've already posted PRC contact info in the post above. Pls contact them directly. There's a customer service booth near the entrance so they can give you the appropriate form and tell you which window to go to. Hope this helps.
DeleteMaam, gud pm need clarificatio a year ago i renewed my dormant prc license for almost 6 years, and also have it converted as electronics engr. from previous ECE. I followed all the requirements and paid all my dues and penalties as well as filled up the panunumpa form. eventhough i complied those requirements way back then as a new ece a few years passed. As far as i know once your former ECE licensed was converted to as electronics engineer it also goes automatically that your board certificate will be replaced with a new one bearing the newly converted title. Now my problem is i was issued with a new converted prc card but the board certificate is not yet available. The irony is when i inquired again with the releasing counter they ask about the Board resolution, is that so when in fact it is only a conversion meaning you have already complied all the requisites way back then thats why i have been issued with this old ECE Board certificate. And i have no idea with that so called board tesolution since all the requisites and process are solely done by PRC correct me if i am wrong, but it is annoying to hear the question askibg yoy about board resolution.

ReplyDeleteIt will depend on what exact document is required, if it says PRC then you need to file it with PRC. I'm not sure if any agency/regulatory authority/institution accepts certifications by lawyers with regards to professional licenses.

ReplyDeleteNo, it's not the same as certificate of good standing. The verification of registration is similar to the board certificate (looks like a diploma). Check PRC website for the procedure under stateboard verification.

ReplyDeleteI'm not sure if they would authenticate if your license is expired. Make sure to schedule your renewal online first before going on your chosen date. Pls refer to the official website for more details.

DeleteYou can attend seminars/conventions for your profession even if you're not a member but it costs a bit more than the member rate.
DeleteIf you lack points, you can still renew your license this year but sign an undertaking that the points you lack will be added to your succeeding license renewal. This is only until December 2018. Please refer to the PRC website for details.

ReplyDeleteYes, I think only the renewal & registration requires an online appointment.
DeleteThe certificate of eligibility is different, it's a document that states that your professional license with PRC is equivalent to the Civil Service Commission's examination. Your PRC license already serves as proof but some companies would require a documentary proof that you're exempted from taking the CSC exam, that's when you apply for the certificate of eligibility.

hello sir, i would like to ask if there is a need to make an special power of attorney if i will authorise someone to request and claim my board certificate and certificate of good standing from PRC.? Im here po kasi abroad and i need it to complete my requirements for IRON program for nurses. I authorised my friend po and i gave my photocopy of my license and my authorisation letter. She also provided her IDs.What happened is that di tinanggap ng PRC yung authorisation letter ko and they want the authorisation letter to be notarised and they are requesting a Special power of attorney pa? Is it necessary po ba to have special power of attorney if meron ng notarised authorisation letter??

Sorry for the really late reply. I guess they've become a bit stricter with the requirements for an authorized representative, especially if they're not your relative. Any transaction to be done in your behalf, especially when it concerns legal documents, would need a special power of attorney. You can inquire with the nearest Philippine embassy or consulate in your area for this.

ReplyDeleteYou could try to ask if they keep a record of the documents being released in the respective window. Just tell them the date you made the request. Better be prepared to apply for another one though, if they refuse to release it to you without your claim stub.

DeleteHi there. Presuming your license is expired, but you have no sufficient cpd units for it to be renewed, can you still apply for a government position which requires eligibility? IF you cant renew, are you still an eligible or what?
ReplyDeleteYou could still apply then just say your prc license renewal is under process.
DeleteFulfill the CPD units to renew your license or sign the undertaking found in the PRC website (that the units you need to complete will be added to the next 3 years' requirement eg. 27+45) so you can renew your license.
Hi. I hope you can help me. When I went to PRC Pagadian to get my board certificate, they told me that is not in their file. I passed the NLE last December 2011 and I registered in Pagadian City so I don't know why my board certificate is not there. They told me that they have to make some follow ups from PRC Zamboanga to check if my certificate was sent there. When I asked them for any turn around time, they can't even provide a time frame. They told me to just wait. I'm just alarmed since my batchmates already have their certificates. Do you know of any incident like mine wherein a board certificate is nowhere to be found? Any explanation on that? I really want to get my board certificate asap since I am applying for a job abroad. Help please. Thank you.

DeleteTry calling them again after 2-3 business days to follow up or go back and ask to see someone higher who can help you. Ask for their telephone number and give your contact info so you can communicate directly.
I have no idea why they wouldn't have it on file if you registered there.

ReplyDeleteAppointments are only for license renewal and registration. Requests for certificates & authentication can be done by walk-in.

Yes, have your documents authenticated by PRC then bring them to DFA for red ribbon/authentication or ask at PRC if they can send the documents directly to DFA for processing.

I can fill out the action sheet, and provide a notarized affidavit of loss, but my problem is if I can ask my dad to process it on my behalf? In this case, I need SPA / would a notarized authorization letter suffice? My dad is PRC registered. How about the 2x2 passport photo? Do I still need to provide that? Hoping for some enlightenment. ReplyDeleteA 2x2 photo is a different size from a passport photo. Your dad can show his PRC ID.

Hello po. I lost my Board Rating, the blue paper sent from PRC via mail. Ang sabi po ng PRC Dumaguete, hindi nadaw po ako makakakuha ng same paper kasi once lang daw po sila mag.rerelease nun. Ibang format na ang 2nd copy. Sad to say, yun pong original ang isa sa mga requirements na kailangan ko. Need help po. Paano po makakakuha ng Board Rating na kaparehas sa Original. Salamat po.

No expiration afaik but it also depends on what the hospital/company will require. Minsan gusto nila current year issued yung certificate.

Di naman sila specific. You should ask the company you're submitting it to kung anong size ng document ang need nila. Saka may photocopiers naman dun sa loob at sa tapat ng PRC.

As I have stated in the blogpost above, Certification lang po ang available for online appointment sa PRC. If you need authentication, walk-in po directly sa PRC branch (no need for online appointment).

ReplyDeletePwede naman if you have an old one. Depende na lang sa institution na pagsu-submitan mo ng document if tatanggapin nila yung older one. Just make sure your full name as written in the document is the same (eg. surname change after marriage, etc.). Hope this helps.
